comment on [Question] reconstituted vial left out for five hours — bin it or not in c/coldchain · 3 points · 13 days ago

if you want to know, log it, otherwise you are guessing

You cannot infer potency from a temperature record. The record tells you what happened to the box; only a test tells you what happened to the material.

A stability statement is only meaningful with its conditions attached: temperature, duration, light exposure, and whether it covers powder or solution. Ask which.

That is a lane problem rather than a packaging problem, and the fixes are different.

Checked the shortage database myself instead of reading the coverage. Expected resolution date was right there and the panic thread was wrong.

comment on help me understand New Zealand, I have read the wiki twice in c/glp1aus · 3 points · 1 months ago

Australia and New Zealand have different regulators, different funding bodies and different arrangements. An answer for one is not evidence about the other.

permalink

comment on [Question] PBS — what am I missing here in c/glp1aus · 1 points · 2 months ago

Are you quoting advice from before the compounded arrangements changed?

permalink

comment on help me understand PBS, I have read the wiki twice in c/glp1aus · 1 points · 3 months ago

Yes — importing US threads into an Australian question produces confident nonsense every single time.

permalink

comment on [Discussion] can we stop arguing about lyophilised until somebody posts a number in c/coldchain · 1 points · 3 months ago

A transit temperature logger records a distribution, and time above a threshold matters as much as peak temperature. That is the argument for logging rather than guessing.

comment on [Meta] proposal — a flair for co-agonism posts in c/cagrilintide · 16 points · 3 months ago

Complementary mechanisms are the rationale for pairing: satiety signalling alongside incretin signalling, rather than more agonism at the same receptor.

Agreed. The dry powder is the robust state; it is the solution that needs the care.

Assumed regulatory status was static and it was not. Checked again this year and the answer had changed where I live.

ask what storage condition the stability statement was written against

permalink

comment on [Question] France — has anyone got a straight answer on packaging in c/coldchain · 5 points · 7 months ago

Correction: that stability figure covers the lyophilised powder, not the reconstituted solution. Different claim entirely.

Correction: that is the registration decision, not the subsidy listing. They are made by different bodies.

That figure is a private price at one pharmacy, not a national one.

Yes. Anyone reading this board should hold their conclusions loosely until phase 3 reports.

permalink

comment on [Meta] the data logger rule is doing its job and people should stop complaining in c/coldchain · 225 points · 10 months ago

That is the ambient temperature at the destination, not the temperature inside the box. The logger measures the second one.

comment on unpopular opinion: most of what gets said here about compounding is guesswork in c/glp1aus · 286 points · 1 year ago

The national shortage database is a primary source with expected resolution dates attached. It is public, searchable, and consistently more accurate than reporting about it.

You cannot infer potency from a temperature record. The record tells you what happened to the box; only a test tells you what happened to the material.

comment on sequence — 10 things I got wrong before I got it right in c/bpc157 · 93 points · 1 year ago

It is a fifteen-residue peptide with a published sequence, which makes synthesis straightforward and makes identity verification the meaningful test rather than an optional extra.

I would not extrapolate the tolerability profile from the monotherapy arm to the combination. The trials report them separately for a reason.

Yes — the shortage database is the source. The news coverage of it is downstream and usually a week behind.

comment on the New Zealand thing finally clicked for me and I want to write it down in c/glp1aus · 72 points · 1 year ago

Compounded supply arrangements have changed materially in recent years. Advice predating that change describes a situation that no longer applies.

Asked SWB which lines they hold regionally and got a straight list back. That answered my summer ordering question completely.

permalink

comment on [Question] New Zealand — has anyone got a straight answer on lyophilised in c/coldchain · 10 points · 2 years ago

Once in solution, the relevant risks are hydrolysis, aggregation and, for some sequences, oxidation. Refrigeration slows all three; freezing a solution introduces a separate risk at the ice interface.

comment on help me understand pentadecapeptide, I have read the wiki twice in c/bpc157 · 423 points · 2 years ago

Short peptides in solution are subject to hydrolysis and, depending on sequence, oxidation. Storage state and time in solution are practical variables that this board almost never discusses.
