comment on FLOW got published and nobody in here is talking about eGFR slope in c/trialwatch · 154 points · 1 days ago

which makes it one of the few readouts where I do not have to demand the number needed to treat, because it is small enough to be obviously worth it.

would need something like an A/B/A design, back to single dosing for a stretch and see if sides come back, to actually separate the two effects, which is a big ask to expect from self-experimentation

reproducibility beats resolution if you only get one of them

priya_ogunleye is right — the integration choice is a decision and it should be stated alongside the result.

would love a version of this table with actual sample sizes attached, "likely noise" vs "likely real" without an n behind it is still a bit of a vibe even if a well-reasoned one

Not convinced. Interval stretching changes your exposure profile, not just the total, and the two are not equivalent.

Gave the same trace to two people I trust and got two integrations about a point apart. That was the most educational afternoon I have had here.

Different laboratories use different assay platforms with different calibration.

Adding the obvious one — take it to whoever ordered the panel. This board cannot read it for you.

Disagree with reading anything into that. One value, one morning, and you changed two other things in the same window.

Took a baseline the week before starting purely because this board told me to. Best five minutes I have spent on this.

Small fix — 214nm, not 210. It matters for the comparison you are making with the other run.

Agreed. The trend is the signal. A single value is a snapshot of one morning.

Why the evidence base here is thin, which is not your imagination.

The major trial programmes were designed around endpoints that do not answer most of the questions this board asks: cycle interactions, contraception, fertility, perimenopause. The populations were not enrolled to answer them and the endpoints were not chosen to measure them.

So when you search and find nothing, that is usually the actual state of the literature rather than a failure of searching. What follows practically: be precise about your question, take the pharmacology parts to a pharmacist who can look up product-specific information, and treat confident answers on this board — including the ones that agree with you — with more caution than usual.

Reference intervals are typically the central 95% of a reference population. By construction one person in twenty falls outside one on any given test without anything being wrong.

Internal appeals are decided by the plan.

adaeze_cabrera is right that this is documentation rather than persuasion. It took me a year to accept that.

Added ApoB to the panel after a thread here. It told me something the standard lipid panel had been hiding.

External review, which is the most underused mechanism discussed on this board.

Internal appeals are decided by the plan. External review sends the determination to an independent body. Where it applies its decision binds the plan, and the deadlines to request it are short and strictly enforced.

The two things that trip people up: not knowing it exists, and exhausting the internal levels so slowly that the external window closes. Ask on the first denial what the external route is and what the deadline will be. Availability and rules vary by jurisdiction and plan type, so say where you are when you ask here.

The habits from the losing phase are what carry it now.

This reframing is the most useful thing on the board for anyone approaching this phase.

comment on copay — 2 things I got wrong before I got it right in c/insurancefights · 10 points · 4 months ago

The process that has actually worked for people on this board, in order.

Get the denial in writing and find the criterion it names. Request the clinical policy bulletin by its number. Write the appeal against that document, criterion by criterion, attaching what has been tried and for how long. Note the deadline from the letter date and diarise it.

If the first level fails, go to the second. If the second fails, ask about external review, which is independent and, where it applies, binding. Keep every date, department and reference number as you go.

It is administrative rather than rhetorical, and the people who win are the ones who treat it that way.

Substantial weight loss independently moves lipids, liver enzymes and several other markers.

Agreed — and standardising the draw conditions is the cheapest improvement available.

comment on [Discussion] lowest effective dose is doing more work than we give it credit for in c/maintenancephase · 31 points · 6 months ago

The two levers, described properly, because this board mixes them up constantly.

Reducing the dose lowers average exposure while keeping the weekly rhythm. Stretching the interval keeps the dose but lowers the trough between injections, which — with a week-long half-life — is why people notice appetite returning towards the end of a stretched cycle before they see anything on the scale.

The reported pattern here is stretching in small steps, a week to ten days to twelve, with appetite as the signal rather than the scale. Some people find dose reduction more comfortable. Neither is a schedule anybody here can hand you, and the fact that this phase has the least published guidance is exactly why board consensus deserves less weight than usual.

ApoB counts atherogenic particles rather than the cholesterol they carry, which is why it can diverge from LDL-C and why it is the addition most worth making.
