comment on three years of copay threads, summarised so you do not have to read them in c/insurancefights · 3 points · 10 months ago

Missed a deadline because I counted from when I opened the envelope. That mistake cost me an entire cycle.

potency testing on the finished preparation is the thing to ask for

comment on Sweden: external review, and what it actually costs here in c/insurancefights · 70 points · 10 months ago

Appeal deadlines run from the date on the determination letter. They are strict, they are short, and a missed deadline usually forfeits that level entirely.

Biopsy-confirmed population or imaging-selected?

not approved anywhere, and the boards forget that constantly

Agreed — and the endpoint definitions are where the actual claim lives.

Disagree with the tone strategy. Anger has never moved a determination; matching the criterion has.

Correction: that is the imaging endpoint, not the histological one. The paper reports both and they differ substantially.

Assumed the weight numbers were the point and was corrected. They are secondary to what this is being developed for.

Internal appeals are decided by the plan. External review is decided by an independent body and, where it applies, its determination is binding. They are separate mechanisms and the second is chronically underused.

hair shedding follows the weight loss, not the drug, and it stops

Correcting my own comment above: I said day three and my log says day two. Same pattern, wrong day.

if it is severe or persistent that is a clinician question, not a board question

Small fix — the shedding follows the rate of weight loss, not the compound, and it is temporary.

Phase 2 in a biopsy-confirmed population is a demanding design: recruitment is slow, the population is specific, and the results do not generalise to people who have not been characterised that way.

comment on [PSA] compounding is not what most of this community thinks it is in c/compounding · 61 points · 1 year ago

The shortage list is the legal hinge: the permissions that allow certain compounding to happen at scale are tied to a drug’s shortage status, which changes.

That symptom is not the one being described upthread. Two different things with similar names and the answers are not interchangeable.

Constipation is mostly reduced intake plus reduced fluid, not a direct effect. That is why the boring fibre and water answer keeps working.

Agreed on the plan-year point. Criteria that applied last year may simply not apply now.

permalink

comment on why does nobody talk about survodutide in c/survodutide · 21 points · 1 year ago

This. Biopsy-confirmed and imaging-suggested are different evidentiary categories and get run together constantly.

smaller meals, earlier, is the boring advice that works

permalink

comment on [Discussion] the copay advice in here is 2 years out of date in c/insurancefights · 15 points · 2 years ago

Agreed on the plan-year point.

Disagreeing with this line: the deadline runs from the letter date and treating it otherwise is expensive.

Correction: that is a known effect of reduced intake rather than a direct drug effect. The distinction matters because the fix is different.

That advice is jurisdiction-specific and this board spans several. Say where you are.

Nothing in this thread is medical advice, and the choice between arrangements is one for you and a prescriber who knows your history.

permalink

comment on someone explain prior authorization to me like I have not read a paper in years in c/insurancefights · 1 points · 2 years ago

Small fix — external review is independent of the plan.

Adding the underused one — external review. Independent, binding where it applies, and hardly anybody gets that far.

Potency and sterility testing on the finished preparation are separate from any certificate covering the starting material. Ask which you are being shown.

eating slower does more than most of the supplements people recommend

Adding to this — holding the dose rather than stepping is the option people forget they have.

permalink

comment on [Discussion] heart rate is doing more work than we give it credit for in c/sideeffects · 845 points · 2 years ago

Severe or persistent abdominal pain sits outside the pattern this board can usefully discuss. The honest answer there is that it needs someone who can examine you.

comment on nobody warned me about fatigue and I would have liked the warning in c/sideeffects · 262 points · 2 years ago

Mine settled at week 55 and has not come back since, across two more steps.
