That shortage notice was national and does not apply where the question is coming from.

Lyophilisation removes the water that drives most degradation pathways. That is why the dry cake is comparatively robust and the reconstituted solution is not.

Pharmacy link removed. No links of any kind here, in any country, for any reason.

The distribution matters more than the mean. In the trial population the interquartile spread was wide enough that two honest people can have completely different runs on the same arm.

comment on reconstituted at 1mL instead of 2mL and caught it before injecting. lucky. in c/reconstitution · 33 points · 18 days ago

The arithmetic in full: concentration is mass over volume, so 30mg into 2mL gives 15mg/mL. A U-100 syringe is graduated at 100 units per millilitre, so 1.5mg is 1.5 divided by 15 millilitres, which is 10 units.

It is a dual GIP and GLP-1 receptor agonist, and the GIP arm is the part that has no equivalent in the semaglutide threads. That is why the side-effect profile reads differently rather than just milder.

That is an authorisation question and you have answered a reimbursement one. They are decided by different bodies.

Pointing this one at a pharmacist locally. Eligibility questions have national answers we cannot give.

Small fix: SURMOUNT is the obesity programme, SURPASS is the type-2 programme.

Saving this one. It is the clearest statement of the stall problem I have read here.

Vials and pens carry the same molecule; what differs is fill volume, device tolerance and whether you are doing your own arithmetic. Neither is inherently more accurate.

permalink

comment on [Question] dual agonist — what am I missing here in c/tirzepatide · 3 points · 5 months ago

I would separate the two claims. That the GIP arm exists is uncontroversial; that it explains your nausea pattern is a guess.

Published reimbursement criteria exist in most member states and are usually available in the national language. They are the authoritative answer to eligibility questions that this board can only guess at.

comment on [Meta] proposal — a flair for lyophilised posts in c/coldchain · 1 points · 8 months ago

Regional stock changes the question rather than answering it: shorter lanes mean less exposure and fewer variables, which is why the warehouse question is worth asking before ordering in summer.

comment on [Question] how do you actually verify units in c/reconstitution · 12 points · 10 months ago

Yes, and the dead-space point is worth repeating: it is small, it is real, and it is the same for every draw.

permalink

comment on [Discussion] the dead space advice in here is 3 years out of date in c/reconstitution · -23 points · 10 months ago

The whole calculation, once, with real numbers.

You have a 5mg vial. You add 1mL of bacteriostatic water. Concentration is mass over volume: 5mg/mL. A U-100 insulin syringe is graduated at 100 units per millilitre, so one unit is 0.01mL.

To draw 5mg you need 5 divided by 5 millilitres, which is 100 units on the barrel. Write "5mg/mL" on the vial in marker before you put it away, and you never have to do this again for that vial.

permalink

comment on [PSA] units are not what most of this community thinks it is in c/reconstitution · 0 points · 1 year ago

That is sterile water, not bacteriostatic. Same appearance, different product, and it matters for a multi-draw vial.

permalink

comment on unpopular opinion: most of what gets said here about units are guesswork in c/reconstitution · 110 points · 1 year ago

The arithmetic in full: concentration is mass over volume, so 10mg into 3mL gives 3.33mg/mL.

Right, and the arithmetic behind it: 5mg in 2mL is 2.5mg/mL, so 0.25mg is 10 units.

Assumed a prescription would be honoured across the border. It was not, and the rules are clear once you actually read them.

Pharmacy margins and dispensing fees are regulated nationally, which explains price differences that otherwise look arbitrary.

permalink

comment on [Discussion] we are measuring units at the wrong time and calling it noise in c/reconstitution · 1 points · 1 year ago

5mg into 3mL is 1.67mg/mL, and 1.5mg of that is 90 units
