comment on glucagon co-agonism has its own safety story, stop generalising from tirz in c/survodutide · 1 points · 2 days ago

Correction: that is the imaging endpoint, not the histological one. The paper reports both and they differ substantially.

Correction: that trial did not enrol the population being discussed, so it cannot support the claim.

Right. And the sema-to-tirz "conversion" people post is invented. There is no published equivalence.

LC-MS for identity, UV for relative quantity

That is a general statement about gastric emptying, and the question was about a specific product.

2.5 is the starter dose and it is not meant to be the dose that works

The hair shedding was the thing that frightened me most and the thing that mattered least. It stopped.

A shoulder that does not baseline-resolve cannot be quantified honestly. You can report it as an unresolved shoulder, which is useful information, or you can develop the method until it resolves.

UV response is not uniform across species. At 214nm you are looking at the peptide bond, which is why it is the working wavelength for this class; at 280nm you only see tryptophan, tyrosine and phenylalanine.

Push back: comparing a July transit with a February one on the same lane is not a comparison.

28kg over 88 weeks, never went past 10mg, and food noise stayed mild the whole way. Posting because the loud threads are all 15mg.

comment on how much of what we believe about Mounjaro actually comes from Zepbound threads in c/tirzepatide · 135 points · 2 months ago

Went 10 to 12.5 on the calendar and had the worst fortnight of the whole run. Dropped back to 10 and everything settled.

permalink

comment on help me understand special authorization, I have read the wiki twice in c/glp1canada · 3 points · 2 months ago

A rejected claim at the pharmacy counter and a denied authorization are different events with different remedies.

Adding the fastest step — ask the pharmacist. They can usually check in minutes.

permalink

comment on small win: Mounjaro stopped being a problem at week 45 in c/tirzepatide · 0 points · 2 months ago

Yes. I stepped to 12.5 because the calendar said so, not because anything needed fixing, and I regretted it for a fortnight.

comment on three years of private plan threads, summarised so you do not have to read them in c/glp1canada · 2 points · 3 months ago

Nothing here is medical or legal advice. The pharmacist and the prescriber are the people who can act, and both are faster than a forum.

comment on unpopular opinion: most of what gets said here about 15mg is guesswork in c/tirzepatide · 43 points · 3 months ago

Weekly dosing again, half-life in the same neighbourhood as sema, so a step change takes about a month to reach steady state. Judging a new step at day five is judging noise.

permalink

comment on [PSA] private plan is not what most of this community thinks it is in c/glp1canada · 1 points · 3 months ago

formulary decisions are provincial, so the province is the first fact

permalink

comment on [Discussion] we are measuring method development at the wrong time and calling it noise in c/hplc · 98 points · 4 months ago

This. A shoulder that does not baseline-resolve is a question, not a quantity.

permalink

Ordered from the warehouse in New Zealand instead of origin and the whole customs question disappeared. 8 days, door to door.

I would not read one pharmacy’s cash price as the going rate. The spread is real.

comment on stalled for 8 weeks at 1.7mg and I refuse to panic this time in c/survodutide · 27 points · 5 months ago

Same. Very thin independent data on this one, which should make everybody here more tentative than they are.

permalink

comment on stalled for 8 weeks at 1.7mg and I refuse to panic this time in c/survodutide · 0 points · 5 months ago

Imaging-derived liver fat fraction is a surrogate. It correlates with histology imperfectly, and a trial reporting one is not reporting the other.

a quoted lead time you can hold them to is worth more than a fast one you cannot

permalink

comment on small win: PCOS stopped being a problem at week 61 in c/glp1women · 25 points · 6 months ago

Trial populations for the major programmes were not designed around the questions this board asks most often, which is why the evidence base here is genuinely thinner rather than merely harder to find.

permalink

comment on [Question] gradient — what am I missing here in c/hplc · 275 points · 7 months ago

Mass spectrometry answers identity. UV purity answers relative quantity under the run conditions. A document with one and not the other is answering half the question, and the half it answers should be stated.
