comment on [Question] appeal — what am I missing here in c/insurancefights · 3 points · 1 year ago

Careful — the deadline runs from the date on the letter. Waiting for a call back can cost you the appeal entirely.

Push back: your plan is an employer plan, which changes both the appeal path and who the regulator is.

Which country and which plan year are we talking about?

comment on [Discussion] the goal weight advice in here is 2 years out of date in c/maintenancephase · 35 points · 1 year ago

Appetite returning before the scale moves follows from the mechanism: the appetite effect is the proximal one and weight is the lagging integrated outcome.

if you are going to test, test the thing you actually received

Correction: that is a formulary exclusion, not a prior authorisation denial. Different form, different route, different deadline.

Yes — the comparison threads are apples to oranges and they generate more heat than anything else here.

That reading assumes area percent and net peptide content are the same number. They are not, and the report says which it is.

comment on why does nobody talk about regain in c/maintenancephase · 223 points · 1 year ago

That reads like a taper schedule and this board cannot hand those out.

comment on [Meta] the tier 3 rule is doing its job and people should stop complaining in c/glp1uk · 91 points · 1 year ago

Read the MHRA notice rather than the news coverage and the actual scope was much narrower than the panic threads suggested.

the MHRA supply notices are published, read them before the panic threads

Four pharmacies, four prices, same product, same month. The spread was larger than I expected and none of them were the headline offer.

comment on [Discussion] can we stop arguing about appeal until somebody posts a number in c/insurancefights · 121 points · 1 year ago

Asked for the policy bulletin by number and wrote the appeal against its criteria line by line. Approved on the second level after a flat first-level denial.

comment on stalled for 3 weeks at 0.5mg and I refuse to panic this time in c/maintenancephase · 790 points · 2 years ago

Maintenance is the phase with the least published guidance and the most individual variation, which is precisely why board consensus here is worth less than usual.

Push back: a private prescription is a separate route, not a shortcut through the NHS queue.

permalink

comment on someone explain prior authorization to me like I have not read a paper in years in c/insurancefights · 65 points · 2 years ago

Internal appeals are decided by the plan. External review is decided by an independent body and, where it applies, its determination is binding. They are separate mechanisms and the second is chronically underused.

That criterion is from the previous plan year. The current bulletin has different wording.

nothing here is available as a prescription product, so read every thread with that in mind

comment on UK — 2 things I got wrong before I got it right in c/glp1uk · 12 points · 2 years ago

A private prescription is an independent route: it does not affect NHS list position, and it is not a step in the same pathway.

Agreed. The ICB variation is the thing that makes national advice useless here.

Inter-lab variance on this assay runs roughly a point either way, driven mostly by gradient and integration. Two results inside that band agree; they do not disagree quietly.

comment on [Discussion] we are measuring below spec at the wrong time and calling it noise in c/labresults · 242 points · 2 years ago

Chain of custody is why member-funded testing is worth more than a supplied certificate: the vial tested is the vial that arrived at a member’s door.

Date of analysis anchors the result to a manufacturing period. A two-year-old result is evidence about two years ago and should be read that way.
