That delay was at clearance, not with the carrier. The lane table separates them for exactly this reason.

New plan year, entirely new criteria, and the denial that had been immovable in the autumn simply did not apply in January.

saskia_lokken is right that this is documentation rather than persuasion. It took me a year to accept that.

Discharge from a specialist service at goal is a normal end point of that pathway. Whether prescribing continues in primary care afterwards is a local decision and is worth asking about at the start rather than at the end.

Internal appeals are decided by the plan. External review is decided by an independent body and, where it applies, its determination is binding. They are separate mechanisms and the second is chronically underused.

Phase 2 estimates effect and finds a dose range. Phase 3 estimates it precisely in a bigger population and catches what phase 2 was too small to see. Treating them as the same tier of evidence is the recurring error here.
