the second-level appeal is where things actually turn

That advice is jurisdiction-specific and this board spans several. Say where you are.

comment on why does nobody talk about ACHIEVE in c/orforglipron · 4 points · 19 days ago

Careful — purity methods for peptides do not read across to a small molecule and the numbers are not equivalent.

permalink

comment on unpopular opinion: most of what gets said here about step therapy is guesswork in c/insurancefights · 1 points · 27 days ago

Not convinced. That is a formulary exclusion rather than a prior authorisation denial, and the route to challenge it is different.

503A is patient-specific, 503B is outsourcing facility, they are not the same thing

comment on help me understand non-peptide, I have read the wiki twice in c/orforglipron · 19 points · 1 months ago

Holding off on any opinion until phase 3 reports.

This is the fact the whole board hangs on. Small molecule, not peptide.

Push back: the ranking is not hiding anything. The formula is published and you can reproduce the ordering.

The no-timing-restriction thing is the part I would care about most in practice, and it barely gets mentioned here.

Disagree with the panic framing.

Adding the practical version: ask which lines are held regionally before you order in July.

That figure is the starting material purity, not the finished preparation potency. Two different tests.

Kept a log with every date, name of department and reference number. When they claimed no record of a call, I had the reference.

if a clinic will not name the facility, that is your answer

Yes — a logger in the box turns a fortnight of speculation into a chart.

comment on does 503B actually matter or is it forum lore at this point in c/compounding · 25 points · 3 months ago

The shortage list is the legal hinge: the permissions that allow certain compounding to happen at scale are tied to a drug’s shortage status, which changes.

permalink

comment on [Question] how do you actually verify step therapy in c/insurancefights · 29 points · 4 months ago

Prior authorisation criteria are republished each plan year. A criterion that blocked you in one year may not exist in the next, so a denial is worth retesting after the turnover.

permalink

comment on how much of what we believe about packaging actually comes from stability threads in c/coldchain · 2 points · 4 months ago

Careful. "The cold chain failed" is a strong claim and the evidence you have is a warm box.

permalink

This. Step therapy is a paperwork requirement and it is beaten with documentation, not persuasion.

keep every date, every reference number, every name of a department

Disagreeing with this line: the deadline runs from the letter date and treating it otherwise is expensive.

comment on help me understand formulary, I have read the wiki twice in c/insurancefights · 6 points · 5 months ago

step therapy is a documentation problem, not an argument

permalink

comment on [Discussion] telehealth is doing more work than we give it credit for in c/compounding · 21 points · 5 months ago

Did they name the facility?

permalink

comment on [Discussion] telehealth is doing more work than we give it credit for in c/compounding · 52 points · 5 months ago

Correction: patient-specific refers to the prescription, not to a bespoke formulation. Common misreading and it changes the argument.

permalink

comment on does denial actually matter or is it forum lore at this point in c/insurancefights · -6 points · 6 months ago

Step therapy requires documented trial of preferred alternatives. It is not an argument to be won on merits; it is a record to be produced, and the record is what the appeal must contain.

permalink

comment on [Question] non-peptide — what am I missing here in c/orforglipron · 16 points · 7 months ago

This. The absence of food and water timing restrictions is the practical difference people will actually notice.

Moved everything off the fridge door after reading a thread here and my temperature log immediately got boring.

permalink

comment on why does nobody talk about excursion in c/coldchain · 25 points · 9 months ago

Ordered from the warehouse in France instead of origin and the transit went from a fortnight to 9 days.

Disagreeing with this line only: freezing a reconstituted vial is not a safe default.

permalink

comment on three years of ranking threads, summarised so you do not have to read them in c/meta · 13 points · 9 months ago

Small fix — archiving happens at a year, not at six months. It is in the published policy.

permalink

comment on three years of copay threads, summarised so you do not have to read them in c/insurancefights · 12 points · 10 months ago

Right, and keeping every date and reference number turns a frustrating process into an auditable one.

permalink

comment on [Meta] the data logger rule is doing its job and people should stop complaining in c/coldchain · 147 points · 10 months ago

Once in solution, the relevant risks are hydrolysis, aggregation and, for some sequences, oxidation. Refrigeration slows all three; freezing a solution introduces a separate risk at the ice interface.
