comment on MASH — my 24-week log, condensed into one table in c/survodutide · 0 points · 5 months ago

The escalation schedules in the published protocols are slow and deliberate, and the tolerability tables show why. That is a description of the trials rather than a suggestion for anybody.

Right, and keeping every date and reference number turns a frustrating process into an auditable one.

modmail first, always, and the thread second

This is the principle the whole site is built on. Publish it or do not enforce it.

Correction: that is the imaging endpoint, not the histological one. The paper reports both and they differ substantially.

A certificate with a purity figure and no method line is not falsifiable. You cannot reproduce it, you cannot check it, and it tells you nothing about how the number was produced.

Mild pushback: a supplier with fewer entries is not worse, it is less sampled. Those are different claims and the table says so.

Prior authorisation criteria are republished each plan year. A criterion that blocked you in one year may not exist in the next, so a denial is worth retesting after the turnover.

New plan year, entirely new criteria, and the denial that had been immovable in the autumn simply did not apply in January.

permalink

comment on does goal weight actually matter or is it forum lore at this point in c/maintenancephase · 15 points · 10 months ago

Maintenance is the phase with the least published guidance and the most individual variation, which is precisely why board consensus here is worth les

Disagreeing with this bit: a two-kilo range is not drift and treating it as a problem creates one.

comment on [Results] 28 weeks on 10mg, full numbers, ask me anything boring in c/survodutide · 32 points · 11 months ago

It is a dual agonist at the GLP-1 and glucagon receptors. The glucagon arm is associated with increased energy expenditure and with effects on hepatic fat, which is why the development programme is oriented towards liver disease.

permalink

comment on [Question] how do you actually verify maintenance in c/maintenancephase · 30 points · 11 months ago

Cosigning on planning it in advance. Deciding what maintenance looks like while still losing is much easier than deciding it in a panic.

Correction: that is a formulary exclusion, not a prior authorisation denial. Different form, different route, different deadline.

comment on genuine question about MASH that I am slightly embarrassed to ask in c/survodutide · 72 points · 1 year ago

Yes — nothing containing this is approved anywhere, which is the first fact and usually the last one mentioned.

permalink

comment on genuine question about dose stretching that I am slightly embarrassed to ask in c/maintenancephase · 67 points · 1 year ago

Body weight varies by a couple of kilos over days in stable people through fluid, glycogen and gut content. A range is not a trend.

permalink

comment on help me understand goal weight, I have read the wiki twice in c/maintenancephase · 32 points · 1 year ago

Small fix — you have described interval stretching and called it a dose reduction. The exposure profiles differ.

permalink

comment on the testing question that gets asked weekly, answered properly in c/vendorvetting · 18 points · 1 year ago

Sent the same compound from QSC and WWB to VendorInvestigate in the same month. Both came back above the claim on the certificate — mine was 97.2% on a stated 97.0%.

The habits from the losing phase are what carry it now. That is not inspirational, it is just what happened.

Not convinced by the ranking-by-vibe approach here. The community number is derived from reports; your feeling is derived from four posts you remember.

Independent testing is worth more than a supplied certificate for one structural reason: the chain of custody. A member who buys, ships and pays is testing the same thing you would receive.

This is the whole method in one sentence.

Agreed — and the corollary is that "no data" and "bad data" have to be written differently. The table separates them for exactly that reason.

The thing that decided it for me was that QST answered a column-and-gradient question in writing without asking why I wanted to know.

Continuous monitoring shows the shape: postprandial excursions, overnight behaviour and time in range. Two people with identical A1c can have very different distributions.

Chased the last three kilos with a dose increase and got a fortnight of injection-site soreness and no change in the number.
