The shortage list is the legal hinge: the permissions that allow certain compounding to happen at scale are tied to a drug’s shortage status, which changes.

Are we talking about receptor affinity or clinical potency?

Are the related substances itemised or is it a single total?

A 503A pharmacy compounds for an identified patient against a prescription. A 503B outsourcing facility registers with the regulator, may produce without patient-specific prescriptions, and is subject to current good manufacturing practice requirements. The two are governed differently and the difference is not cosmetic.

Not quite. Two labs with different gradients can resolve a close-eluting impurity differently and both be reporting honestly.

Area percent or net peptide content? They answer different questions.

Asked GL Biochem for the batch-specific certificate and had the full document — batch, date, method line naming the column — inside six hours. That is now my baseline for everybody.

Small fix — 214nm, not 280nm. It matters here because the impurity you are asking about barely absorbs at the longer wavelength.

Glucose-dependent insulin secretion is why hypoglycaemia risk is low as monotherapy: the effect scales with glycaemia rather than acting unconditionally.

comment on why does nobody talk about state board in c/compounding · 253 points · 2 years ago

Small fix — 503B facilities register with the regulator; 503A pharmacies are licensed by the state board. Different mechanisms.

Is that from a human study or a preclinical model?

Disagreeing with this specific inference — that is a preclinical result being read as human pharmacology.

Yes — and the reason to ask for the whole page is that the interesting parts are the ones people crop out to fit a screenshot.

a 503B has to register and report, so there is a paper trail to ask for

Correcting myself upthread: I said the date was missing and it is on the second page, which I had not scrolled to.

Disagree on this specific point. Two honest labs can resolve that impurity differently and report different numbers.
