comment on unpopular opinion: most of what gets said here about units are guesswork in c/reconstitution · 98 points · 1 year ago

the powder cake sits at the bottom, the volume you add is the volume that counts

Potency and sterility testing on the finished preparation are separate from any certificate covering the starting material. Ask which you are being shown.

I would not panic about that.

Agreed, and for anyone finding this later: that is the normal experience, not a warning sign.

That is area percent from a certificate, not the amount of peptide in the vial. Two different numbers and a very common mix-up.

Same view. The second-level appeal is where mine turned, after a first-level denial that looked final.

comment on switched from 0.5mg to 2.4mg and early fullness got better, not worse in c/newbiequestions · 176 points · 1 year ago

Spent my first fortnight comparing myself to the loudest posters. Do not do that; they are not the middle of the distribution.

comment on what changed for me between month 11 and month 16 in c/newbiequestions · 26 points · 1 year ago

Appetite change and weight change are different measurements on different timescales. The first is usually noticed first and the second lags it.

comment on nobody warned me about sulphur burps and I would have liked the warning in c/sideeffects · 5 points · 1 year ago

Careful with the supplement recommendation. There is no evidence for it here and the mechanism you are proposing does not hold.

nobody warns you about how much of this is timing rather than dose

comment on genuine question about denial that I am slightly embarrassed to ask in c/insurancefights · 14 points · 1 year ago

That criterion is from the previous plan year.

hassan_chowdhury is right that this is documentation rather than persuasion. It took me a year to accept that.

comment on the constipation question that gets asked weekly, answered properly in c/sideeffects · 1 points · 1 year ago

Agreed on writing it down. I could not see the pattern until I had six weeks of it on one page.

permalink

comment on reflux — 5 things I got wrong before I got it right in c/sideeffects · 210 points · 1 year ago

The effect on gastric emptying is most pronounced early and attenuates with continued dosing, which is the mechanism behind the "it settles at a stable dose" pattern the whole board reports.

if the maths gives you a fraction of a unit, change the concentration

Spent six weeks arguing in general terms and got nowhere. Two paragraphs quoting their own criteria turned it around in eleven days.

comment on someone explain insulin syringe to me like I have not read a paper in years in c/reconstitution · 103 points · 1 year ago

The arithmetic in full: concentration is mass over volume, so 15mg into 3mL gives 5mg/mL. A U-100 syringe is graduated at 100 units per millilitre, so 1.5mg is 1.5 divided by 5 millilitres, which is 30 units.

Took three weeks to understand titration and about ten seconds once somebody explained it as a tolerance ladder.

the first month is a tolerance check, not a results phase

Did the maths give you a whole number of units, or a fraction?

Bacteriostatic or plain sterile water?

Are you accounting for the powder volume, or is that where the confusion is?

Wrote the date on the vial as well as the concentration after losing track of which one was opened first.

A denial letter is required to state a reason and to reference the criterion applied. That reference is the handle: request the clinical policy document by its identifier and answer it point by point.
