comment on mg ÷ mL, that is the whole formula, why did i find this hard in c/reconstitution · 6 points · 23 hr ago

How many mg in the vial and how many mL are you adding?

permalink

comment on [Regulatory] 503A vs 503B in one paragraph, because everyone conflates them in c/compounding · 26 points · 2 days ago

a 503B has to register and report, so there is a paper trail to ask for

permalink

comment on foam means you shook it. do not shake it in c/reconstitution · 101 points · 2 days ago

What concentration did you actually reconstitute at, rather than what did you intend to?

permalink

comment on [Explainer] why 1mL syringes cause overdoses and 0.3mL ones do not in c/reconstitution · 7 points · 3 days ago

Small fix — a U-100 syringe is 100 units per mL. If yours is marked differently it is not U-100 and the whole calculation changes.

comment on week two, nothing is happening, everyone says that is normal, is it in c/newbiequestions · 15 points · 4 days ago

Took three weeks to understand titration and about ten seconds once somebody explained it as a tolerance ladder.

Shook the first vial because nobody told me not to. It was fine, but I have swirled every one since.

comment on [Appeal] the external review process nobody tells you exists in c/insurancefights · 0 points · 5 days ago

Agreed — and request the bulletin by number.

This is the whole method. Answer the criterion they named, not the decision in general.

permalink

comment on [Log] public log, started week 1, currently week 90, still updating in c/dosinglogs · 3 points · 7 days ago

Right, and the payoff arrives around month six when a pattern becomes visible that nobody could hold in their head.

permalink

comment on muscle cramps turned out to be sodium, not magnesium in c/sideeffects · 6 points · 10 days ago

nobody warns you about how much of this is timing rather than dose

permalink

comment on [Discussion] the compounding gap was a symptom, not a solution in c/compounding · 154 points · 11 days ago

Switched between two compounded preparations and the concentration on the label was different. Redid the arithmetic on paper before drawing anything.

permalink

comment on cloudy after reconstitution — discard, right? right. in c/reconstitution · 17 points · 12 days ago

Agreed — units are volume. Once that lands, the rest of the arithmetic is one division.

permalink

comment on [PSA] let the alcohol swab dry before you puncture, that is the step everyone skips in c/reconstitution · 11 points · 16 days ago

Same. I got this wrong once, caught it because the units looked absurd, and have written on every vial since.

permalink

comment on [Failure] vendor vanished mid-order with 11 people waiting in c/darkspot · 63 points · 18 days ago

Contemporaneous notes matter most in exactly the situations where people are least inclined to write them. Memory reorganises a bad fortnight into a tidier story with a clearer villain.

comment on [Appeal] peer-to-peer review: what to say and what to bring in c/insurancefights · 1 points · 20 days ago

keep every date, every reference number, every name of a department

permalink

comment on [Regret] told my whole family, then stopped, then had to tell them that too in c/darkspot · 13 points · 21 days ago

What would have caught it earliest?

permalink

comment on [Appeal] peer-to-peer review: what to say and what to bring in c/insurancefights · 12 points · 21 days ago

Small fix — external review is independent of the plan. The second-level internal appeal is not.

permalink

comment on unpopular opinion: most of what gets said here about log is guesswork in c/dosinglogs · 25 points · 1 months ago

the value of a log is in month six, not week one

permalink

comment on 23 months in and week one is still the thing I get wrong in c/newbiequestions · 9 points · 1 months ago

a COA is a certificate of analysis, the lab document for a batch

permalink

comment on [Discussion] the bacteriostatic water advice in here is 3 years out of date in c/reconstitution · 3 points · 1 months ago

more water is not more drug, it is the same drug in a bigger volume

permalink

comment on [Discussion] the bacteriostatic water advice in here is 3 years out of date in c/reconstitution · 58 points · 1 months ago

Small fix — a U-100 syringe is 100 units per mL. If yours is marked differently it is not U-100 and the whole calculation changes.

permalink

comment on small win: dose history stopped being a problem at week 86 in c/dosinglogs · 5 points · 2 months ago

annotate the weeks where something else changed

permalink

comment on reading what is threads from 2024 and half of it aged badly in c/newbiequestions · 13 points · 2 months ago

Small correction, said kindly: those are two different products with similar names, and the difference matters for a multi-draw vial.

more water is not more drug, it is the same drug in a bigger volume

What exactly does the denial letter give as the reason?

permalink

comment on prior authorization — 22 things I got wrong before I got it right in c/insurancefights · 208 points · 2 months ago

Push back: your plan is an employer plan, which changes both the appeal path and who the regulator is.

permalink

comment on [Results] 94 weeks, 34kg, and waist was the hard part in c/dosinglogs · 3 points · 2 months ago

This. Annotating the weeks where something else changed is what makes the log interpretable later.

permalink

comment on unpopular opinion: most of what gets said here about what went wrong is guesswork in c/darkspot · 12 points · 3 months ago

Is this the first time, or a pattern you can see now?

permalink

comment on how much of what we believe about units actually comes from dead space threads in c/reconstitution · 0 points · 3 months ago

Bacteriostatic water contains benzyl alcohol as a preservative, which is what makes repeated entry into the same vial reasonable. Plain sterile water has no preservative.

permalink

comment on my triglycerides moved and I cannot work out whether what is is why in c/newbiequestions · 16 points · 3 months ago

the first month is a tolerance check, not a results phase

permalink

comment on compounding is the most under-discussed thing on this board in c/compounding · 9 points · 3 months ago

do not assume the concentration matches the branded product

permalink

comment on [Question] how do you actually verify dead space in c/reconstitution · 0 points · 3 months ago

Correction: that gives 5 units, not what you have written.

Adding one thing — put the date on the label too. Concentration plus date and you can never mix two vials up.

comment on unpopular opinion: most of what gets said here about external review is guesswork in c/insurancefights · 8 points · 3 months ago

This. Step therapy is a paperwork requirement and it is beaten with documentation, not persuasion.

permalink

Prior authorisation criteria are republished each plan year. A criterion that blocked you in one year may not exist in the next, so a denial is worth retesting after the turnover.

comment on stalled for 14 weeks at 12.5mg and I refuse to panic this time in c/darkspot · 33 points · 3 months ago

Ignored something for four days that I should have asked about on day one. That is the only genuine regret in my whole run.

permalink

comment on anyone else notice failure kicking in around week 21 in c/darkspot · 11 points · 3 months ago

money lost to a shop with nothing on file is the recurring story

permalink

comment on anyone else notice failure kicking in around week 21 in c/darkspot · 123 points · 3 months ago

Agreed. The failures are the part that is missing from every other board, which is why this one matters.

permalink

comment on how much of what we believe about 503A actually comes from telehealth threads in c/compounding · 1 points · 4 months ago

Compounded preparations are not approved products and carry no bioequivalence claim. That is a statement about regulatory category, not about quality.
