comment on [Meta] proposal — a flair for appeal posts in c/insurancefights · 25 points · 1 year ago

Appeal deadlines run from the date on the determination letter. They are strict, they are short, and a missed deadline usually forfeits that level entirely.

Correction: that study measured regain after discontinuation, not loss of effect during treatment. Very different findings.

Agreed, and the trial number is a mean of a very wide distribution — the tails are enormous and nobody posts from the middle.

That advice is jurisdiction-specific and this board spans several. Say where you are.

Small fix — external review is independent of the plan. The second-level internal appeal is not.

Careful with "everyone tolerates it better". The people it did not suit stop posting, which makes this board look calmer than the drug is.

Are you comparing yourself with the trial mean or with the people who post the most?

25kg over 64 weeks, never went past 10mg, and early fullness stayed mild the whole way. Posting because the loud threads are all 15mg.

Right. And the sema-to-tirz "conversion" people post is invented. There is no published equivalence.

comment on sequence: what the trials say vs what this community says in c/bpc157 · 137 points · 1 year ago

Push back: "no reported harms" in a literature with almost no human trials is not a safety finding.

A denial letter is required to state a reason and to reference the criterion applied.

elise_grimaldi is right that this is documentation rather than persuasion. It took me a year to accept that.

comment on [Discussion] we are measuring regain at the wrong time and calling it noise in c/tapering · 320 points · 1 year ago

Withdrawal studies across this class consistently show substantial regain after discontinuation, with appetite typically returning before weight changes. That is a description of the pharmacology of a chronic condition.

That percentage is body weight change, not body fat. Different measurement, and the distinction gets lost every time.

Correction to my own post above — I said 2.5mg and I have been on 15mg since the spring. Same argument, wrong number.

Have you held a step for longer than the four-week minimum at any point?

Held 7.5 for five months. Everyone kept asking when I was going to 10. The answer was never, because 7.5 was working.

Careful — that is a question for whoever ordered the panel, and nobody in this thread can answer it responsibly.

My ApoB moved and I spent a fortnight convinced I knew why. The repeat six weeks later was back in range and I had changed nothing.

comment on appeal — 4 things I got wrong before I got it right in c/insurancefights · 95 points · 2 years ago

Prior authorisation criteria are republished each plan year.

Adding the underused one — external review. Independent, binding where it applies, and hardly anybody gets that far.

Agreed. Province first — coverage is a provincial decision and national answers are wrong somewhere by construction.

Spent six weeks arguing in general terms and got nowhere. Two paragraphs quoting their own criteria turned it around in eleven days.

A peer-to-peer conversation puts the prescribing clinician in front of a reviewing clinician. It bypasses the correspondence cycle entirely and is often the fastest available route.

Small fix: SURMOUNT is the obesity programme, SURPASS is the type-2 programme. They are different endpoints and the numbers do not transfer.

Sulphur burps for the first eight days after each step, then nothing. Predictable enough that I planned my week around it.
