and note what this does to your reconstitution arithmetic: if you calculate concentration off the label weight you are slightly over-estimating. it is usually within the noise of everything else, but it is not zero.

yeah I see that exact error pattern constantly, someone reconstitutes differently "because they ran low on bac water" and just keeps using the old unit count

for anyone following along and confused why 0.05mg bumps are even measurable: it comes down to your reconstitution concentration. mg in the vial divided by mL added gives you mg/mL, and a fine enough concentration makes a 0.05mg step land on a clean, readable unit mark on a standard insulin syringe. get the concentration wrong and these small steps become guesswork.

That only works if the fill weight is exactly the label weight, and it usually is not exactly anything.

On the goal weight you set in week one.

You chose it with the least information you will ever have about how this feels, what is sustainable, and what your body does at different points. Revising it is not moving the goalposts; it is updating a prediction, which is what you are supposed to do with predictions.

Several of the longest-standing members here have revised theirs more than once, in both directions, and describe the revision as the point at which the whole thing became manageable. What the number should be is a conversation with somebody who knows your history — but that it is allowed to change is something this board can tell you.

The failure modes I have actually seen posted here, in order of frequency.

One: reconstituting at a different volume than intended and not writing it down, so the next draw is based on a remembered concentration. Two: treating units as a dose rather than a volume, which breaks the moment the concentration changes. Three: assuming a syringe is U-100 when it is not.

All three are solved by the same two habits — do the arithmetic before the water goes in, and write the result on the glass. Neither costs anything and between them they cover almost every question this board gets.

Correction: that gives 100 units, not what you have written. 10mg into 2mL is 5mg/mL and the division follows from there.

This. Decide in advance what you are watching for. Deciding in the middle of it is much harder.
