comment on why does nobody talk about copay in c/insurancefights · 16 points · 1 year ago

Step therapy requires documented trial of preferred alternatives. It is not an argument to be won on merits; it is a record to be produced, and the record is what the appeal must contain.

That reading assumes area percent and net peptide content are the same number. They are not, and the report says which it is.

Glucagon receptor agonism is associated with increased energy expenditure and with hepatic effects. That is a mechanistic story with strong preclinical support and limited phase 2 human data.

comment on [PSA] cagrilintide is not what most of this community thinks it is in c/cagrilintide · 507 points · 1 year ago

Independent purity data on this compound is sparse compared with the older molecules, simply because far fewer members have paid for testing. Sparse data means wide uncertainty, not a verdict.

the phase 2 numbers were striking and they were also 48 weeks in a small population

Choose a format you will still be able to open in two years. Plain text or a spreadsheet you export beats an application that may not exist.

New plan year, entirely new criteria, and the denial that had been immovable in the autumn simply did not apply in January.

This is the whole method. Answer the criterion they named, not the decision in general.

the second-level appeal is where things actually turn

A log is only interpretable if the conditions are consistent. Same day, same time, same state, same scale — otherwise you are recording measurement variability rather than change.

Agreed that consistent conditions matter more than frequency. Daily weights under varying conditions are noise.

Draw at the same point in the week now, fasting, same lab. The noise dropped enormously once I standardised.

Push back: the evidence base here is thin enough that a confident ranking against the established compounds is not supportable.

comment on [Results] 57 weeks, 25kg, and glucagon was the hard part in c/retatrutide · 281 points · 1 year ago

Agreed, and the framing that helps is: this is a phase 2 compound with phase 3 running. Everything else is inference.

Documented eighteen months of what had been tried in a one-page table. That table was the appeal.

A token symbol identifies an asset, not a network.

This is the check that prevents the only genuinely unrecoverable mistake in this thread.

I would not extrapolate the tolerability profile from the monotherapy arm to the combination. The trials report them separately for a reason.

Asked for the policy bulletin by number and wrote the appeal against its criteria line by line. Approved on the second level after a flat first-level denial.

Increases in heart rate have been reported across this receptor class. The magnitude and clinical significance are exactly what phase 3 is powered to establish.

Yes — date of analysis, because a result from two years ago describes a different manufacturing month.

That service reports area percent by default. Worth stating, since the thread has been comparing it to a mass figure.

permalink

comment on why does nobody talk about test results in c/labresults · 15 points · 2 years ago

The template is vendor, compound, batch, service, claim, result, and who paid. Every one of those fields exists because a result missing it has been argued about here before.
