comment on am I the only one who found orforglipron harder than the injections in c/orforglipron · 26 points · 11 months ago

It is a non-peptide small-molecule agonist at the GLP-1 receptor. That is a substantial medicinal chemistry achievement: getting a small molecule to activate a receptor evolved for a peptide ligand.

a non-peptide agonist does not degrade the way a peptide does

That criterion is from the previous plan year. The current bulletin has different wording.

The press release said that; the publication says something more hedged. Worth reading both.

Yes. The starting dose is a tolerance step. It is not supposed to do very much.

Agreed on the swirl. There is no reason to shake it and a good reason not to.

comment on [Question] how do you actually verify DEXA in c/glp1muscle · 24 points · 1 year ago

Careful, that is a nutrition prescription aimed at another member and this board does not do those.

resistance training first, cardio if there is time left

comment on [PSA] units are not what most of this community thinks it is in c/reconstitution · 6 points · 1 year ago

Cosigning the round-number approach. Reconstituting to something awkward is how you end up drawing 27.5 units at six in the morning.

comment on what changed for me between month 3 and month 6 in c/orforglipron · 13 points · 1 year ago

Daily dosing gives a very different exposure profile from a weekly injectable: peaks and troughs within each day rather than a smoothed weekly curve.

Asked what a COA was in my first week. Somebody linked the explainer and I have been reading them ever since.

Careful with that mean. The distribution around it was wide enough that it describes very few individual participants.

comment on tried protein for 24 weeks. here is what happened. in c/glp1muscle · 1 points · 1 year ago

Disagree — slower loss does not automatically preserve lean mass. Protein and training do the work, not the rate on its own.

Hydration status and fasting state affect the lean mass estimate. Standardising both is the difference between a comparison and a coin toss.

Research-use-only material is not approved for human use. That is a plain statement of regulatory fact and it belongs in every beginner thread.

permalink

comment on [Question] appeal — what am I missing here in c/insurancefights · 4 points · 1 year ago

Not convinced. That is a formulary exclusion rather than a prior authorisation denial, and the route to challenge it is different.

comment on [Meta] the Apotheke rule is doing its job and people should stop complaining in c/glp1eu · 15 points · 1 year ago

Disagree. That is one member state’s reimbursement rule and it does not generalise across the union.

On cross-border prescriptions, since the assumption behind half these threads is wrong.

Adding the practical step — read your own country’s published criteria. They exist and they are definitive.

Open-label extensions lose their randomisation. Anybody still enrolled at week 104 is a selected group and the numbers describe that group.

Peer-to-peer took fifteen minutes and resolved something a written appeal had been sitting on for a month.

Push back on the advice above: nobody here can tell a member what dose to take, however confidently it is phrased.

Adding the one habit worth starting today: write the concentration on the vial.

Disagree with daily weighing. You are logging fluid, and the emotional cost is real for very little information.

Not quite.

trialwatch_theo has explained this better than the wiki does, which is saying something.

Kept a log with every date, name of department and reference number. When they claimed no record of a call, I had the reference.

Employer-sponsored plans may be regulated differently from individually purchased ones, which changes both the appeal route and which regulator hears a complaint.

Disagreeing with this line: the deadline runs from the letter date and treating it otherwise is expensive.

Strength retention is an imperfect but free longitudinal proxy: it is affected by neural factors and by being in a deficit, so it tracks tissue change

mariam_cabrera is right that protein becomes a logistics problem when appetite drops.
