The NHS route in England runs through specialist weight management services commissioned by your ICB. Criteria, waiting times and what happens at discharge are all local decisions, which is why a confident answer from somebody in a different area may be wrong for you.

The private route is an independent transaction: a consultation, a prescription and a pharmacy price. It does not affect your position on any NHS list and it is not a shortcut through one. Both are legitimate; they are simply not the same pathway and they do not connect.

Pharmacy pricing on a private prescription is set by the pharmacy. The spread between them in a given month is routinely substantial, which is why a tracker built from member reports is more useful than any headline.

If a supplier holds the line you want in a warehouse in your own region, your order is a domestic shipment. No international leg, no clearance, a shorter cold exposure and a lead time in days rather than weeks. If they do not, it is an international shipment however it is described at checkout.

So the question is not "how fast do you ship" but "which lines do you hold in regional stock, and is the one I want among them". It is one email, the answer is usually a list, and it changes what to order and when — especially in summer.

Internal appeals are decided by the plan. External review sends the determination to an independent body. Where it applies its decision binds the plan, and the deadlines to request it are short and strictly enforced.

The two things that trip people up: not knowing it exists, and exhausting the internal levels so slowly that the external window closes. Ask on the first denial what the external route is and what the deadline will be. Availability and rules vary by jurisdiction and plan type, so say where you are when you ask here.

The arithmetic in full: concentration is mass over volume, so 5mg into 1mL gives 5mg/mL. A U-100 syringe is graduated at 100 units per millilitre, so 2mg is 2 divided by 5 millilitres, which is 40 units.

The vial contains a fixed amount of peptide. Water changes nothing about that amount. What it changes is concentration, and concentration is the divisor between the dose you want and the volume you draw.

So doubling the water halves the concentration and doubles the units for the same dose. Nothing is diluted in any sense that matters; you are simply reading a bigger number off the same barrel. Pick the water volume that puts your usual dose on a whole number of units and life gets easier.
