comment on nobody warned me about early fullness and I would have liked the warning in c/maintenancephase · 423 points · 2 years ago

Dose reduction and interval extension are different interventions. Halving the dose keeps the rhythm and lowers the level; stretching keeps the level and lowers the frequency, with a lower trough.

comment on the reship thing finally clicked for me and I want to write it down in c/vendorvetting · 98 points · 2 years ago

Kept a spreadsheet of quoted versus actual lead time across 8 orders. The quoted number held.

plan the maintenance phase before you need it

I would not read across from the obesity programmes. Different population, different endpoint, different question.

Postprandial excursions typically respond earlier than fasting glucose, so the sequence people observe is not a sign that something is not working.

Yes. Spread across lots is the number that tells you about the process. A single high result tells you about a vial.

comment on someone explain prior authorization to me like I have not read a paper in years in c/insurancefights · 15 points · 2 years ago

Is this a prior authorisation denial or a formulary exclusion?

ferran_batista is right that this is documentation rather than persuasion. It took me a year to accept that.

Stretching the interval, reducing the dose, or both?

Small fix — you have described interval stretching and called it a dose reduction. The exposure profiles differ.

Stretched from seven to ten days and then to twelve over about four months. Appetite told me where the limit was before the scale did.

What dose are you holding, and for how long now?

comment on [Question] testing — what am I missing here in c/vendorvetting · 300 points · 2 years ago

Independent testing is worth more than a supplied certificate for one structural reason: the chain of custody. A member who buys, ships and pays is testing the same thing you would receive.

MASH endpoints are histological, which is a much harder bar

comment on vendor is the most under-discussed thing on this board in c/vendorvetting · 207 points · 2 years ago

Six orders across two years from QYB. Same packaging, same paperwork, same lead time. The most boring supplier relationship I have and I mean that as praise.

comment on unpopular opinion: most of what gets said here about denial is guesswork in c/insurancefights · 104 points · 2 years ago

Disagree with the tone strategy. Anger has never moved a determination; matching the criterion has.

comment on does external review actually matter or is it forum lore at this point in c/insurancefights · 18 points · 2 years ago

That advice is jurisdiction-specific and this board spans several. Say where you are.

Lead time and minimum quantity are the two facts people forget to ask for and then argue about later. Both are answerable in one email before you commit.

Is the goal weight still the one you picked at the start?

comment on [Question] step therapy — what am I missing here in c/insurancefights · 1.2k points · 2 years ago

A denial letter is required to state a reason and to reference the criterion applied. That reference is the handle: request the clinical policy document by its identifier and answer it point by point.

reship policy in writing before the order, not after

comment on [Discussion] can we stop arguing about rules until somebody posts a number in c/meta · 205 points · 2 years ago

A rule-change proposal needs replacement wording because that is what can be evaluated. Without it there is nothing to compare against the current text.

Right — QYB publishes the method line on the certificate, and once you have seen one that does you notice every one that does not.

comment on [Meta] proposal — a flair for wiki posts in c/meta · 17 points · 2 years ago

Vendor source pages carry an outbound link to the supplier’s own storefront, marked nofollow and sponsored, disclosed on the page and again on the funding page. Nothing is received for it.
