comment on the insulin syringe thing finally clicked for me and I want to write it down in c/reconstitution · 1 points · 12 months ago
Reconstituted at 2mL when I meant to use a different volume.
Right, and the arithmetic behind it: 20mg in 2mL is 10mg/mL, so 2mg is 20 units.

if the maths gives you a fraction of a unit, change the concentration
comment on [Discussion] denial is doing more work than we give it credit for in c/insurancefights · 0 points · 12 months ago
Yes — quoting their own policy bulletin back at them is far more effective than arguing in general terms.
Disagreeing with this line: the deadline runs from the letter date and treating it otherwise is expensive.

Same view. The second-level appeal is where mine turned, after a first-level denial that looked final.

Cosigning the domain check. Character by character, every time, and paste it rather than typing it.

Reconstituted at 2mL when I meant to use a different volume. Caught it because the units looked wrong on the barrel. Now I write 10mg/mL on the glass before I put the cap back.

Appeal deadlines run from the date on the determination letter.

Yes — describe the sequence of events with dates. That is a report. Adjectives are not.

Agreed. The denial letter tells you which criterion failed, and answering that specific criterion is the entire job.

The dead space question sent me down a rabbit hole for a week. It is a fraction of a unit and it is the same every draw.
comment on the prior authorization thing finally clicked for me and I want to write it down in c/insurancefights · 7 points · 1 year ago
Right, and keeping every date and reference number turns a frustrating process into an auditable one.
comment on genuine question about units that I am slightly embarrassed to ask in c/reconstitution · 15 points · 1 year ago
Yes, and the dead-space point is worth repeating: it is small, it is real, and it is the same for every draw.
comment on units — 21 things I got wrong before I got it right in c/reconstitution · 27 points · 1 year ago
Correction: that gives 10 units, not what you have written. 30mg into 2mL is 15mg/mL and the division follows from there.

How many mg in the vial and how many mL are you adding?
comment on units — 21 things I got wrong before I got it right in c/reconstitution · 99 points · 1 year ago
Right — the volume you add is the only variable you control here, and the cake does not change it meaningfully.
Adding one thing — put the date on the label too. Concentration plus date and you can never mix two vials up.

twice as much water halves the concentration and doubles the units for the same dose

Lost money once, years ago, to a shop that pushed hard for a decision the same day. Every time I see that pressure now I close the tab.

viktor_kirchner has the units point exactly right.
Right, and the arithmetic behind it: 10mg in 1mL is 10mg/mL, so 2mg is 20 units.

Yes. Writing 10mg/mL on the vial takes four seconds and removes the entire class of error this board exists to fix.

Cosigning on external review. It is a real mechanism, it is underused, and the deadlines are strict.
