Some chains require a memo or destination tag to route funds to the right account. Omitting it on those chains is one of the more common ways payments go astray.

Started keeping a folder of every certificate I have been sent. The ones that itemise impurities are a small group and they are mostly the same suppliers.

Quantity limits restrict how much may be dispensed in a period even where coverage exists.

laila_wojcik is right that a rejected claim and a denied authorization are different animals.

National regulatory approval establishes that a product may be sold. Whether any plan pays for it is a separate decision, and public drug plans are administered provincially.

A rejected claim at the pharmacy counter and a denied authorization are different events with different remedies.

Step therapy requires documented trial of preferred alternatives. It is not an argument to be won on merits; it is a record to be produced, and the record is what the appeal must contain.
